Information you enter here

The Contact page collects your name, email address, any optional phone number, your state, inquiry topic, and any optional scheduling message. FormSubmit processes the submission and emails it to therapist@amygreenlmft.com. Do not include diagnoses, detailed health history, or other sensitive information. The form is not a secure clinical intake portal; it does not confirm an appointment or establish care. The website describes an optional Personal Reflection Journal that may be shared during therapy; it does not collect journal entries or attachments on these informational pages.

Personal Reflection Journal

The public website describes this optional tool without providing direct access. If it is appropriate and you would like to use it, Amy may share it during therapy and explain its privacy, saving, and sharing arrangements before use. Journal entries are not submitted through this website’s Contact form.

Therapy fit guide

The fit guide processes the text and preferences you enter locally in the browser using keyword rules. Its custom code does not transmit entries to an AI service, Amy, analytics, or the Contact form, and does not store them in browser storage. Entries and results stay in page memory until cleared or the page is left or refreshed. Use Clear my entry, especially on a shared device. Avoid including identifying information or personal health history. Ordinary hosting-related technical processing still applies.

Website preferences

The “Less imagery” preference is stored in your browser’s local storage under “amy-calm.” It helps remember your display choice and does not include health information. You can remove it by clearing this site’s browser data.

Hosting & technical information

The website’s hosting service may process technical information needed to deliver and protect the site, such as your IP address, requested pages, and browser information. This website’s custom code does not add advertising trackers or analytics scripts. Hosting-related processing is governed by the hosting provider’s terms and privacy practices.

Contact form processing

The contact form uses FormSubmit for delivery and spam screening. Submission information passes through that service and is delivered by email to the practice. Review FormSubmit’s privacy information before submitting. This website does not save submitted inquiries to a local contact database. Information received by the practice is handled through its applicable communication and privacy practices.

External services

Links to the original practice inquiry page, SimplePractice, crisis resources, and other services take you to external websites. Those services have their own privacy practices. Review them before entering personal information.

Clinical privacy

This website policy is separate from the practice’s Notice of Privacy Practices and clinical consent documents. Clinical privacy information, permitted disclosures, and applicable patient rights are provided through the practice’s intake process. Request those documents before beginning care.

Privacy questions

Use your established practice contact method, or the practice inquiry options, to request the clinical privacy notice or ask questions. Keep sensitive information out of unverified communication channels.

This notice will be updated if the site adds a contact form, analytics, client accounts, or other data-processing features.
